By using EMRRX, you consent to receive healthcare services via telemedicine.

• Electronic communication between you and a healthcare provider
• Review of your medical history and health information
• Possible diagnosis and treatment recommendations

You understand that:

• Telehealth may have limitations compared to in-person visits
• Technical issues may occur
• Not all conditions can be treated via telemedicine
• Providers may determine that in-person care is required

3. Provider Relationship

Medical services are provided by licensed healthcare professionals through affiliated provider groups, including Ola Health.

These providers are responsible for:

• Medical evaluations
• Clinical decisions
• Prescriptions (if appropriate)

EMRRX does not control or influence medical decisions.

4. Prescriptions & Treatment

• Prescriptions are issued only when medically appropriate
• Not all patients will qualify for treatment
• Providers may decline to prescribe based on clinical judgment
• Certain medications, including controlled substances, may not be prescribed

5. Pharmacy Fulfillment

Prescriptions may be fulfilled by licensed U.S. pharmacies, including Ola Pharmacy.

Pharmacies are responsible for:

• Dispensing medications
• Shipping and delivery
• Compliance with pharmacy regulations

EMRRX does not dispense medications.

6. Eligibility

By using this platform, you confirm that:

• You are at least 18 years old (or meet legal requirements)
• The information you provide is accurate and complete
• You are located in a state where services are permitted

7. Payment & Fees

• Consultation fees are charged for provider evaluations
• Medication costs are separate and vary by treatment and pharmacy
• All payments are final unless otherwise stated

8. No Emergency Services

EMRRX does not provide emergency medical services.

Call 911 immediately or go to the nearest emergency room

9. Privacy & Data Use

Your personal and health information is handled in accordance with our Privacy Policy and applicable laws, including HIPAA.

10. Limitation of Liability

To the maximum extent permitted by law:

• EMRRX is not responsible for medical decisions made by providers
• EMRRX is not responsible for medications dispensed by pharmacies
• EMRRX provides a platform and does not guarantee outcomes

11. State Availability

Services are available only in states where affiliated providers are licensed to practice.
