By requesting laboratory services through EMRRX, I acknowledge and agree to the following:

Test Request and Clinical Review: I voluntarily request the laboratory test(s) selected at checkout. My request is subject to availability, clinical appropriateness, and applicable federal and state requirements. I understand that an appropriately licensed healthcare provider or authorized ordering entity may review my request and may approve, modify, or decline it. Payment does not guarantee approval of a laboratory order.

Service Providers: EMRRX facilitates access to laboratory services but does not collect specimens, perform laboratory testing, or independently diagnose or treat medical conditions. PanelRoute facilitates the ordering workflow. Laboratory testing and specimen collection are performed by participating laboratories and collection centers. Clinical services, when required, are provided by appropriately licensed healthcare professionals or authorized ordering entities.

Information Sharing: I authorize EMRRX to transmit the personal, contact, health, order, and payment-related information reasonably necessary to coordinate and provide the requested services to PanelRoute, the applicable ordering provider or entity, participating laboratories, specimen collection centers, payment processors, and other service providers involved in fulfilling my order. Information will be handled as described in the applicable Privacy Policy and Notice of Privacy Practices and as permitted or required by law.

Laboratory Collection and Testing: I agree to follow all preparation and collection instructions. Specimen collection may involve discomfort, bruising, bleeding, dizziness, fainting, infection, or the need for recollection. A specimen may be delayed, rejected, lost, damaged, insufficient, or unsuitable for testing. In such circumstances, recollection, cancellation, or additional processing may be required, subject to the applicable refund and recollection policy.

Results and Clinical Limitations: Laboratory testing alone is not a comprehensive medical evaluation and does not establish or exclude every diagnosis. Results may be normal, abnormal, inconclusive, falsely positive, or falsely negative. Abnormal or unexpected results may require consultation with a qualified healthcare professional, additional testing, treatment, or in-person medical care.

I am responsible for reviewing my results and completing any recommended follow-up. I understand that I should not delay seeking medical care while waiting for an order or result.

Critical or Emergency Conditions: EMRRX laboratory services are not intended for medical emergencies and are not continuously monitored. If I believe I am experiencing an emergency, I will call 911 or seek immediate emergency care. Critical results may be communicated to the ordering provider and to me using the contact information I provide.

Privacy Notice: I acknowledge that I have been given electronic access to the applicable Privacy Policy and Notice of Privacy Practices. This acknowledgment confirms receipt or availability of those notices and does not waive my privacy rights or authorize unrelated uses of my health information.

Pricing, Payment, Cancellation, and Refunds: I have reviewed the displayed price for my selected test(s) and the applicable cancellation, expiration, recollection, and refund policy. I understand that the purchase is self-pay unless expressly stated otherwise and that insurance reimbursement is not guaranteed. Any additional service that requires a separate charge will be disclosed before I agree to purchase it.

Service-Related Communications: I consent to receive communications necessary to administer my requested services, including identity verification, order confirmations, collection instructions, appointment information, result notifications, safety information, and essential follow-up by email, telephone, or SMS at the contact information I provide. Message and data rates may apply. SMS messages may contain limited information to protect my privacy. I may reply STOP to discontinue nonessential text messages. This consent does not authorize promotional or marketing communications.

Electronic Transactions and Signatures: I consent to conduct this transaction electronically. I confirm that I can access, review, download, and retain these terms. By selecting the acceptance checkbox and entering my legal name, I intend to provide an electronic signature with the same legal effect as my handwritten signature. I will be provided access to a copy of the accepted terms.
